Role Description

This is a full-time on-site role for a Care Manager based in Chennai. The Care Manager will oversee day-to-day case management, coordinate personalized care plans for individuals, and ensure the well-being of elderly or dependent clients. Responsibilities include assessing client needs, liaising with medical and non-medical professionals, and providing support to families. The Care Manager will also conduct training sessions and ensure the implementation of best practices in elder care.

Qualifications

• Experience in Elder Care and Social Services
• Knowledge in Medicine and healthcare management
• Empathy, excellent communication skills, and the ability to connect with clients and their families
• Problem-solving skills and the ability to manage multiple responsibilities efficiently
• Relevant certification or degree in Social Work, Healthcare, or a related field is preferred
