Job Title: Transition Project Coordinator
Experience: 9+ Years
Work Location: Chennai
Position Overview
The Transition Project Coordinator plays a key role in managing and coordinating all activities required to transition services, applications, or operational responsibilities from one team or vendor to another.
The role works closely with the Transition Manager to ensure effective planning, execution, governance, and reporting, enabling a smooth and controlled handover to steady-state operations.
Key Responsibilities
- Coordinate and track end-to-end transition activities across multiple workstreams.
- Support the Transition/Project Manager in planning, scheduling, and execution of transition tasks.
- Maintain transition plans, task trackers, and milestone schedules.
- Monitor progress, follow up on action items, and highlight delays or blockers.
- Organize and manage transition governance meetings, including preparation of agendas and Minutes of Meeting (MoM).
- Maintain and update RAID (Risks, Assumptions, Issues, Dependencies) logs and escalate risks in a timely manner.
- Coordinate Knowledge Transfer (KT) sessions, including scheduling, tracking attendance, and obtaining sign-offs.
- Support service readiness, cutover, and go-live activities.
- Prepare and distribute status reports, dashboards, and stakeholder communications.
- Support handover to steady-state operations and post go-live hypercare tracking.
Skills & Competencies
- Strong coordination and organizational skills
- Excellent written and verbal communication
- Attention to detail with strong follow-up discipline
- Ability to work with multiple teams and stakeholders
- Experience with project tracking and reporting tools such as Excel, Smartsheet, JIRA, and MS Project (basic level)
- Familiarity with transition, service readiness, and Knowledge Transfer processes
- Understanding of service operations and governance models
