Job Description

Job Purpose

The Regional Dialysis Program Manager provides strategic, operational, and clinical leadership for dialysis services across an assigned region. This leader ensures high-quality patient outcomes, compliance with regulatory requirements, effective staffing, financial stewardship, and alignment with organizational goals. The role partners closely with facility administrators, nurse leaders, physicians, interdisciplinary care teams, and corporate leadership to drive excellence in kidney care delivery.

Essential Functions Include But Not Limited To

  • Incorporates AVEMs mission, vision, values and purpose in designing a dialysis program across the region.
  • Oversee daily operations of the Avem dialysis program within the region, ensuring efficient workflows, working equipment, adequate staffing, and adherence to organizational protocols.
  • Implement standardized practices to drive consistency, safety, and performance across all sites.
  • Lead continuous improvement initiatives to optimize patient safety, dialysis scheduling, and resource utilization.
  • Ensure all facilities maintain full compliance with CMS Conditions of Participation, state regulations, and internal policies.
  • Monitor clinical outcomes and develop strategies to achieve or exceed benchmarks.
  • Support the implementation of evidence-based clinical protocols and best practices in kidney care, hemodialysis, and peritoneal dialysis.
  • Coach, mentor, and develop nurses, providers, and interdisciplinary team members across the region.
  • Partner with HR and nursing leadership on recruitment, retention, competency development, and performance management.
  • Foster a culture of teamwork, psychological safety, just culture, and professional accountability.
  • Support growth strategies, including new program development, service line expansion, and business development opportunities.
  • Ensure a patient-centered approach that supports dignity, safety, communication, and satisfaction.
  • Cultivate strong relationships with patients, caregivers, nephrologists, and the broader healthcare community.
  • Address escalated patient concerns and collaborate on care plans for complex cases.
  • Use dashboards, quality indicators, and operational metrics to assess performance and guide decision-making.
  • Track, report on, and improve key indicators: QAPI measures, staffing patterns, incident reports, clinical outcomes, and financial performance.
  • Lead monthly or quarterly quality and operations review meetings and meet regularly with nephrologists for program continuous improvement.
  • Partner with senior leadership on short- and long-range strategic goals for the dialysis service line.
  • Lead initiatives to improve access to care, expand modalities and enhance operational capacity.
  • Supports due diligence activities for dialysis services during hospital acquisitions and leads start up or transition planning for newly acquired dialysis programs within the region.
  • Provides educational programs to staff and physicians as needed.

  • Adhere to the Avem Health Partners Code of Conduct and Standards of Behavior. Comply with established policies and procedures and all health and safety requirements.

Education/Qualifications

  • Graduate of an accredited school of nursing, BSN preferred
  • 5 years of dialysis experience required, 2+ years in leadership or multi-site management preferred.
  • Travel required across assigned region (up to 25%).
  • On-call responsibilities for escalated clinical or operational issues.

Certification/Licensure

  • Current RN license in the state(s) of practice
  • Current BLS (or obtained within 30 days of hire)
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) at hire or within one year.
