Role Description

This is a full-time, on-site Care Manager role based in the Salt Lake Area. The Care Manager will coordinate and oversee care plans for elderly clients, ensuring they receive appropriate services such as healthcare, household assistance, and social support. The role involves case management, liaising with caregivers, monitoring client needs, and ensuring timely delivery of services, including essential medical supplies and emergency assistance. Additionally, the Care Manager will work closely with families and community resources to ensure holistic support for clients.


Key Responsibilities:

1.Make member home visits and evaluate living conditions

2. Addressing emergencies on war foot basis and any other service required by members.

3. Ensuring timely services to members.

4. Communicating with their respective children who are living abroad.

5. Coordinate with operation team in case of any emergency.

6. Maintain proper communication with members as well as office staffs.

7. Avoid making direct cash transactions with any one

8. Avoid smoking, chewing tobacco or consuming alcohol is strictly prohibited during discharging your duty.

9. Coordinating elder care between multiple service providers & team mates

10. Developing both short and long term care plans

11. Helping manage emotional concerns, stress, and anxieties as and when require

12. Helping manage transitional care

13. Ongoing assessment of aging life care needs

14. Maintaining Health and Hygiene, Maintain attendance & Punctuality


Qualifications

  • Strong experience in Case Management and coordination of care plans
  • Expertise in Elder Care and Social Services, understanding the unique needs of senior citizens
  • Knowledge of Medicine and familiarity with medical supplies and prescriptions
  • Skills in Training and mentoring caregivers
  • Exceptional organizational and communication skills, with the ability to collaborate effectively with families and service providers
  • Bachelor’s degree in Social Work, Healthcare Administration, or a related field preferred
  • Previous experience in the healthcare or elder care industry is an advantage
