2care.ai builds clinically safe AI workers that automate healthcare administration. Our Voice AI agents answer patient calls, schedule appointments, manage referrals, handle follow ups, and integrate directly with healthcare systems.

Our customers use 2care.ai to automate up to 90% of front desk operations, reduce missed calls, improve patient experience, and recover lost revenue. Everything we build runs in production and is used by real clinics and hospitals every day.

The Role

We're looking for an engineer who enjoys building production Voice AI systems and taking ownership.

You'll design, build, deploy, and improve Voice AI agents that handle real patient conversations. From telephony and backend APIs to cloud infrastructure and production deployments, you'll own features end to end and work directly with customers to solve real problems.

Responsibilities
  • Build, deploy, and maintain production Voice AI agents using Bolna and modern LLMs.
  • Design conversational workflows with interruption handling, barge-in, context management, retries, and escalation logic.
  • Integrate telephony providers including Plivo, Twilio, SIP, and PSTN infrastructure.
  • Build backend services using Python and FastAPI with REST APIs and WebSockets.
  • Develop integrations with EHRs, Practice Management Systems, CRMs, calendars, and third-party APIs.
  • Deploy and maintain AWS infrastructure using Lambda, SQS, RDS, Cognito, S3, SES, DocumentDB, CloudWatch, CloudTrail, and KMS.
  • Optimise streaming latency, prompt engineering, function calling, tool execution, and overall conversation quality.
  • Debug production call flows, audio quality issues, STT/TTS failures, API failures, and infrastructure bottlenecks.
  • Build logging, observability, evaluation pipelines, and monitoring to improve reliability.
  • Own customer deployments end to end and drive features from design through production.
What We're Looking For
  • 3+ years of backend or AI engineering experience and atleast 1+ years in voice ai 
  • 1 year in devops. AWS. Experience deploying applications on AWS and building scalable cloud-native services.
  • Experience building production Voice AI applications using Bolna, Vapi, Retell, LiveKit, Pipecat, or similar frameworks.
  • Strong Python development with FastAPI and asynchronous programming.
  • Experience working with WebSockets, streaming APIs, and real-time event-driven architectures.
  • Experience integrating Twilio, Plivo, SIP, or other telephony platforms.
  • Familiarity with LLM orchestration, prompt engineering, tool calling, RAG, and conversation state management.
  • Strong debugging skills across backend systems, networking, and production infrastructure.
  • Comfortable owning production systems, learning new technologies quickly, and solving ambiguous engineering problems
Tech Stack

You'll work across our production platform using Voice AI: Bolna, OpenAI, Claude, Deepgram, ElevenLabs, and LiveKit. Backend: Python, FastAPI, PostgreSQL, REST APIs, and WebSockets. AWS: Amplify, RDS, Cognito, S3, Lambda, SQS, SES, DocumentDB, CloudWatch, CloudTrail, and KMS. Telephony: Plivo, Twilio, and SIP.
