As a Palliative Care Coordinator, you’ll play a critical role in ensuring patients and families experience seamless, supportive access to palliative care services.

What You’ll Do

Manage and coordinate incoming referrals from hospitals, physicians, and community partners
Ensure all referral documentation is complete, accurate, and processed efficiently
Support insurance verification and prior authorizations in partnership with revenue cycle

Create a Seamless Patient Experience

Schedule initial consultations and follow-up visits with clinical teams
Guide patients and families through the intake process with clarity and compassion
Ensure timely follow-up and communication every step of the way

Connect teams and information

Serve as a key liaison between patients, families, referral sources, and interdisciplinary teams
Facilitate clear, proactive communication to ensure continuity of care
Maintain accurate records in EMR and internal tracking systems

Drive operational excellence

Track referrals, scheduling metrics, and program performance
Identify opportunities to improve workflows and efficiency
Support reporting, team meetings, and program operations

What You Bring

2+ years of experience in healthcare coordination, referral management, patient services, or similar roles
Experience working in EMR systems
Strong organization, communication, and problem-solving skills
Ability to manage multiple priorities in a fast-paced environment
A service-oriented mindset with a compassionate, patient-first approach
Experience in palliative care, hospice, home health, or care coordination
Familiarity with healthcare terminology and referral workflows
Experience working with interdisciplinary healthcare teams
