Role Overview
The PRN Occupational Therapist evaluates, plans, and administers individualized occupational therapy to patients in their homes, focusing on developing, recovering, or maintaining daily living and work skills. Guided by physician orders, the therapist performs assessments, creates and updates care plans, and provides skilled modalities within scope of practice. Key responsibilities include documenting care, participating in case conferences, supervising Certified Occupational Therapy Assistants, and coordinating with healthcare team members. The role emphasizes patient and family education, cultural sensitivity, and adherence to federal and state regulations. The therapist also plans discharges, refers to community resources, and participates in quality improvement and ongoing education activities.

Qualifications & Skills
- Graduate of an accredited Occupational Therapy program or eligible for national registration
- Current Ohio licensure as an Occupational Therapist
- Minimum two years of home care experience; supervisory experience for those overseeing Assistants
- Current CPR certification, health clearance, and valid driver’s license with good record
- Strong communication, collaboration, documentation, and compliance skills
- Commitment to ongoing professional development and confidentiality
