Job Description
Payrate: $18.00 - $21.16/hr.
Summary:
The Clinical Support Coordinator is responsible for coordinating and supporting various activities related to the CalAIM Community Supports services. This role involves reviewing screening documents, processing service authorizations, and ensuring member engagement and retention through effective communication and coordination with internal staff, providers, and community-based organizations.
Responsibilities
- Review CalAIM Community Supports Screening documents to determine member eligibility for services.
- Follow up with internal staff and providers to support successful referral and enrollment of members.
- Process daily service authorizations and denials for requested Community Supports services.
- Perform data entry, tracking, and case documentation for all referred members, including enrollment status and care coordination activities.
- Support with reconciling provider invoicing and claims.
- Facilitate communication with Community Supports Providers to ensure timely submission of deliverables and reports.
- Assist with the implementation of CalAIM Community Supports trainings by providing ongoing education on services and referral processes.
- Identify and address gaps/barriers in the care coordination process to improve member access to services.
- Review Provider reports to ensure accurate data collection and reporting.
- Respond to inquiries regarding service authorization request status.
- Participate in Multidisciplinary Team meetings to coordinate member care and support retention in services.
- Conduct outreach calls to members, as needed, to verify engagement in services.
- Ensure compliance with all state and federal rules and regulations.
- Perform other assigned duties, as required.
Qualifications
- Experience in a clinical support or healthcare coordination role.
- Strong organizational and data entry skills.
- Ability to communicate effectively with internal and external stakeholders.
- Knowledge of healthcare regulations and compliance standards.
Desired Skills
- Familiarity with CalAIM Community Supports services.
- Experience with service authorizations and case management.
- Strong problem-solving skills and ability to address care coordination barriers.
- Proficiency in using healthcare management software and databases.
Pay Transparency: The typical base pay for this role across the U.S. is: $18.00 - $21.16/hr. Non-exempt positions are eligible for overtime at a rate of 1.5 times the base hourly rate for all hours worked in excess of 40 in a work week, or as required by state or local law. Final offer amounts, within the base pay set forth above, are determined by factors including your relevant skills, education and experience. Full-time employees are eligible to select from different benefits packages.
