Role Description

This is a full-time on-site role for a Care Coordinator based in Sahibzada Ajit Singh Nagar. The Care Coordinator will play an essential role in managing patient care and serving as the primary point of contact for patients throughout their surgical journey. Responsibilities include coordinating appointments, managing case details, assisting with administrative tasks such as insurance clearance and admission formalities, and ensuring patients have access to necessary resources, including transportation and post-surgical follow-ups. The Care Coordinator is instrumental in providing a seamless, supportive experience that allows patients and their families to focus on recovery.

Qualifications

• Proficiency in Care Coordination and Case Management
• Strong Communication skills and experience in Customer Service
• Experience in Home Care or similar health-related roles
• Highly organized with the ability to manage multiple cases effectively
• Problem-solving mindset and ability to work collaboratively with teams
• Familiarity with healthcare procedures and patient management systems is a plus
• Minimum educational requirement: Bachelor’s degree in healthcare administration, nursing, or a related field
